Abstract

Introduction: The Singular Therapeutic Project (STP) stands out as an approach centered on co-production and co-management of care, focusing on people or groups in vulnerable situations. It is a primary practice for understanding the complex and subjective demands of a family cared for by the Family Health Strategy team (eESF). Its construction is guided by a collaborative relationship between health professionals, patients and their families, promoting empowerment and strengthening bonds between them, through reciprocal trust. Objective: The objective of this article is to report on overcoming the challenges encountered by a patient with diabetes through STP. Methodology: This work was carried out by academics from the Medicine course at UNIGRANRIO-AFYA, Duque de Caxias (RJ), supervised by their professors, together with the eESF responsible for the micro area of a community in Duque de Caxias, where the reported experiences occurred. It involves the construction of the STP together with a patient who has diabetes and hypertension, is in a wheelchair and has had her left leg amputated below the knee, due to complications from diabetes and does not have a good relationship with eESF. Results: The lack of a good relationship with the eESF guided the establishment of the objectives of subsequent visits to construct the STP goals, which resulted in the collaborative elaboration of short, medium and long-term goals. Conclusion: The STP made it possible for medical students to interact with the eESF and the designated family, providing the opportunity to experience a humanized medical practice carried out through live work, resulting in closer ties between patients and eESF.
